Developing the Format of Your Survey Form

To be most effective, your written survey forms need to be easy to 
fill out -- and look that way too. Phone surveys need to be constructed 
and written for the ear.  Doing so will help increase the response 
rate and prevent errors from being made in answering the questions.  
Don't put aside your design and layout knowledge just because the 
communication piece you are working on is a mere survey form.  
It, too, needs to communicate well. This chapter will help you make decisions about the format of your questionnaire. You will learn how to: • Determine the most appropriate overall look of the survey. • Decide how responses will be recorded. • Structure the range of possible responses to questions, including whether or not to include a 'no opinion' option. • Organize the information on your survey.
